Your plan, your way At the 'click' of a button have complete choice and control! We provide Plan Management for participant's of the NDIS and can proudly say we are an independent Plan Manager. We take our role as an intermediary seriously and want to ensure that participants are maximising on their funding. We are a member of Disability Intermediaries Australia.

Every support worker in Elevate Support Care™ are vetted via background checks, police checks,references and qualification verifications where necessary. Every Support worker in Elevate Support Care™ system has been examined on their experience and qualifications. And they have been reviewed and rated accordingly. You will have access to go through those reviews before you select your support worker.

We provide a wide range of allied health services including child development, chronic disease nursing, counselling, dementia support, dental, dietetics, drug & alcohol services, group exercise & social activities, occupational therapy, physiotherapy and podiatry. Services are available face-to-face (with COVID-safe measures in place) or via Telehealth (phone or video consultation). We offer free and low-cost services for Healthcare and Pension Card holders. NDIS Day Support Programs also are also available at Parkdale and our three other centres across Kingston. Additionally, we offer disability support in the community, 1:1 support at home and tailored online programs. NDIS Support Coordination is also available.
